According to a report presented by the Wall Street Journal on Sunday, the Indonesian authorities have blamed the failures in the design and oversight for the 2018 Lion Air plane crash. The plane crash killed 189 people shortly after it took off from Jakarta last October. The authorities also blamed pilot error and maintenance error for the crash. The Boeing model was grounded worldwide after the Ethiopian Airlines crash.
