DGCA found severe safety lapses with Indigo airlines after a special audit was conducted at Indigo's office in Gurugram, and issued notices to 4 senior airline executives - Captain Sanjiv Bhalla, Captain Hemant Kumar, Captain Ashim Mitra, and Captain Rakesh Srivastava. The audit was carried on July 8 & 9. Due to multiple landing incidents, DGCA is auditing airlines & airports in monsoon-hit areas.
